If you are an eligible non-union employee, you may also purchase optional life insurance:
For yourself in the amount of:
$50,000 or
1 to 4 times your annual salary, to a maximum of $1 million in coverage.
For your spouse or domestic partner in the amount of:
$5,000
$10,000
$25,000 or
$50,000.
Coverage for your spouse or domestic partner can be up to 50% of the amount of your optional life insurance coverage.
For your dependent children in the amount of
$100 for children under 6 months of age
$5,000 per child for children 6 months - 19 years (or 25 years if full-time students).
Be sure to name a beneficiary for your life insurance benefit by completing the Beneficary Designation Form.
If you purchase additional life insurance after you are first eligible, you may be required to complete an Evidence of Insurability (EOI) Form for review and approval by the insurance company.
