LIFE/DISABILITY INSURANCE | BASIC LIFE INSURANCE

If you’re an eligible union or non-union employee, John Muir Health provides you with basic life insurance of:

$25,000 at no cost to you.

Be sure to name a beneficiary for your life insurance benefit by completing the Beneficary Designation Form.

Eligible non-union employees may purchase additional life insurance coverage.
